Request form Use this form to:
Change your address

Change your company’s trading or mailing address.

Important: If the change is due to a change in ownership, a new merchant facility must be established. Please call Merchant Business Solutions sales on 1300 603 266 (between 8am to 6pm, Monday to Friday (excluding public holidays).

Change your authorised contacts

Add or remove an authorised contact from your merchant facility.

Merchant closure request form Request closure of your merchant facility or terminal(s).
Direct debit or settlement account

Request to change your billing or settlement account.

Important: In some instances, it is a requirement that your account settles into a Bank of Melbourne bank account. If your facility falls into this category, you will not be able to change your settlement account.

Mail and Telephone Order Facility (MOTO) Request approval to enable Mail and Telephone Order (MOTO) on your merchant facility.
