Credit Card Balance Transfer Request Form
Existing Bank of Melbourne Credit Card customers can simply complete this balance transfer request form and send it back to us. Or, you can also request a balance transfer by logging in to Internet Banking and selecting “Transfer & Payments” and then “Payout a Credit Card held with another Bank”.
New customers can request a balance transfer as part of the credit card application.

Download, complete and return this form to set up an automatic direct debit from your nominated bank account.

Credit Limit Increase Application Form (personal)
Download, complete and return this form to request an increase personal to your Credit Card limit. To be eligible for a Bank of Melbourne Credit Card limit increase you must be the primary or joint cardholder, not an additional cardholder, and have not requested a credit limit increase in the last six (6) months.

Credit Limit Increase Application Form (business)
Download, complete and return this form to request an increase to your Amplify Business Credit Card limit. To be eligible for a Bank of Melbourne Credit Card limit increase you must be the primary cardholder, not an additional cardholder, and have not requested a credit limit increase in the last six (6) months.

Direct Debit Request / Authority for Automatic Transfer – Get Set Loan
You can set up an automatic payment plan so that your Get Set loan repayment is taken out of your nominated account on a weekly, fortnightly or monthly basis. Just complete this form and fax it back to us.

Home Loan Protection – PDS and Application Form
You can download and read the PDS and complete the attached Application Form to apply. Or you can ask a Bank of Melbourne Lender in your nearest branch.

Home Loan Redraw Request Form
Redraw is generally available on our variable rate home loans. For fixed rate loans, redraw is available only for loans fixed prior to 30 November 2009. Download, complete and return this form to access any additional money that you have paid into your loan, above your standard repayments. Each redraw request is subject to the Bank’s approval. A fee may be payable for each approved request.

Payee Account Information Form
Internet and Phone Banking – Payee Account Information (also known as “Third Party”).
There is a cumulative limit of $5000 per account, per day, on accounts set up via Internet Banking. There is a maximum limit of $25,000 per day. For example: if you have 5 different accounts you can transfer $5,000 to each account per day.
To increase the daily limit to $100,000, please download, complete and return this form. Allow up to 2 business days for the form to be processed.

Access your additional repayments using redraw.

Phone and Internet Banking Loan Account Redraw Facility Registration
For personal and home loans. You can use Internet and Phone Banking to request redraws from your personal or home loan and have the money transferred into your transaction account. If you aren’t already registered, you need to first register for Internet and Phone Banking. Then complete and return this form.
Home loans - note that redraw is available only on variable rate loans and loans fixed prior to 30 November 2009 for up to $10,000 per annum. Each redraw request is subject to the Bank’s approval. A fee may be payable for each approved request.

Proof of Linked Account Ownership – Direct Saver
For online applications only. For security reasons, before linking your non-Bank of Melbourne transaction banking account to your Direct Saver, we need proof that the transaction banking account belongs to you.

Switch Amplify Card Reward Program
Download, complete and return this form to change the rewards program to which your Amplify Card(s) are linked.

Switch of Financial Institution and Account Details (recurring payments only)
You can list your regular payments on this form to advise relevant organisations your new bank account details via fax or mail. You can fax the forms to Bank of Melbourne 1300 132 755. Bank of Melbourne will forward these forms to the relevant organisations on your behalf within 2 business days.

T

Telegraphic Transfer Authorisation / Cancellation Request
Download, complete and return this form to set up the functionality to transfer money to an international bank account via Internet Banking. Or, you can complete this request online through Internet Banking. Please allow up to 2 business days for the request to be processed.
Once processed you will have access to this function the next time you logon to Internet Banking, using the ‘Transfer to an International Account’ function located within the Transfers and Payments menu.
